▍ Going Abroad before the end of this summer vacation :***Please follow in order!***
1. Login to your Google account & fill in the form: https://reurl.cc/lozgjQ
2. Print & fill in the document named "Notice and Affidavit Letter_1110526" attachment below.
3. Send a text (not photo) message by LINE to Ms. Julia the date of your departure & the date of your return to Taiwan.
4. About "Seal of the Department Office" column, ask your advisor(or Dept. Chairman)to sign it first, then come to Ms. Julia.
5. Then submit the signed document or electronic scanning file to Ms. Lin(Daisy) DIRECTLY.
